There is a form on the DVA Website called Medilist & Health Record. This form enables you and/or your GP/Pharmacist to enter & print all of the medications that you are taking as well as brief overview of your health. It is a very handy form to have in your vehicle whilst travelling, or when you have to see a GP that is not your normal doctor. I would advise anyone thinking of travelling to carry one with these forms with then.

Go to www.dva.gov.au then on the right hand side you will see DVA forms, click on this and then go to form number on the left hand side, scroll down to form number D9073, click open. You can now enter your information and print the form from there or print it off and have it completed by your health professional.

If you have difficulty getting this form please email me and I will send you a copy.

It seems like a great idea, but I've set up my own drug audit, and while I'm here in town I only go to the one pharmacy.
When I'm on the road I carry a list from the most recent Doctor I've seen, as well as my PBS record, which they can trace online if there are any anomolies.
I take a heap of serious drugs which need authorities, but with all the tracking they do over what I have dispensed, I have no idea how the addicts sell their supply and get more.
Eventually they must get caught, even if they change doctors and use different pharmacies.
I need all of mine, and I have no intention of selling any of them.
